(ii) any misrepresentation of information may result in the cancellation;
(iii) information placed in my camp record will be used for the purpose of admission, registration, record keeping, statistical research, or program evaluation and for purposes consistent with Executive Spa Group's administration;
(iv) I am subject to the policies and rules of Executive Spa Group.
