The information provided in this form is used to register your legal name and information with Executive Spa Group.

Certificates and recognition will be assigned to the registrant in accordance with the information provided in this document.

 

Executive Spa Group protects your information in accordance with Alberta's Freedom of Information and Protection of Privacy Act (FOIP).

Information is kept private and will not be shared with 3rd parties, unless required to do so for contracting or government funding/ grant purposes.

PAYMENT METHOD

There are 3 ways to pay your deposit:

  1. Pay in person.
  2. E-transfer to info@executivespagroup.com.
  3. Pay by phone with a credit card.

 

TERMS AND CONDITIONS

I understand that I am responsible for all charges of this registration at Executive Spa Group.

I understand that it is my responsibility to read all policies and procedures as outlined by Executive Spa Group.

I understand a course deposit of 50% of the fee is due at registration. There are no refunds for course deposits.

It is my obligation to pay all fees in full for all courses upon arrival on the first day.

DECLARATION

I hereby declare that the information provide is accurate. I agree that the information provided may be used to determine my eligibility for the program and/ or funding.

My application may be denied by the program provider if I do not meet the requirements, in such case I will be provided with a formal response stating the reason.

 I am subject to the policies and rules of Executive Spa Group. I certify that all statements on this application are true and complete.  I authorize Executive Spa Group to verify them.
